Two toxins - toxin A and B have been implicated in the disease caused by C. difficile.
Toxin A: This toxin is classified as an enterotoxin, meaning it specifically targets the intestines, increasing intestinal permeability, which leads to the accumulation of intestinal fluid, resulting in diarrhea. This disruption of the intestinal barrier contributes to the symptoms and severity of the infection.
Toxin B: This toxin is identified as a potent cytotoxin, meaning it has a toxic effect on cells. In cell cultures, Toxin B causes cells to lose their shape and round off, indicating significant cellular damage. This cytotoxic effect contributes to the tissue damage and inflammation seen in infections caused by C. difficile.
The role of toxin B in the development of bowel disease is unclear. However, it has been hypothesized that the two proteins act synergistically in vivo. Some strains also express the binary toxin (encoded by the cdtA / B genes), which damages human cells by inhibiting actin polymerisation.

What is Clostridioides difficile binary toxin (CDT)
Binary toxin (CDT) is frequently observed in Clostridium difficile strains and is associated with an increased severity of C. difficile infection (CDI). CDT belongs to the family of binary ADP-ribosylating toxins consisting of two separate toxin components: CDTa, the enzymatic ADP-ribosyltransferase that modifies actin, and CDTb, which binds to host cells and translocates CDTa into the cytosol.
Multiple clinical studies indicate an association between binary toxin genes in C. difficile and increased 30-day CDI mortality, independent of PCR ribotype.
